Prior To Your Appointment

• New patient paperwork should be emailed to you three days prior to your appointment from our system. Please have them filled out prior to your appointment. If you have not received an email, please text us and let us know. If you prefer to fill your forms out in-office, please arrive 15 minutes prior to your scheduled appointment time to do so.

• Please bring your most recent pair of glasses or your backup glasses if you wear contacts. If you have it on hand, your previous prescription would also provide the doctor with helpful information.

• Masks are currently optional at our clinic, but please let us know if you would prefer for our staff to mask up for your visit. We would be happy to accommodate you!

Where To Find Us + Parking

• We are located at the newly renovated Hill Shopping Center. Parking is available in the front or back lot.

• Coming from the South parking lot off Walnut Hill Ln: We are on the backside of the Bellagreen restaurant. Walk along the corridor between Bellagreen and the Movement Gym. Take a right after passing Hello Dumpling and we will be next door on the right.

• Coming from the North parking lot off I-75: Park behind the CAMP kids store. Walk into the courtyard between CAMP and SynergenX. Walk past Clean Juice and follow the stairs down to see Thrive on your left next to Hello Dumpling.
